How do I submit a claim to GeoBlue?
Where do I get a claim form? Claim forms are available for download on the Member Hub. Once you are logged in, click ‘Claims’ in the top right corner and then ‘How do you file a claim with GeoBlue’. From there you’ll be able to file an eClaim or download a medical claim form.
