Submitting a Life Claim with American Income Life Insurance

Hi all,

My dad passed away back in June 2019. I was unaware he had any type of life insurance, but recently found out that he had a policy with American Income Life Insurance Company (I know I know, I’ve heard bad things). He only had the policy for about a year or so, so I know this process would require some extra things as detailed by their life claim instructions (anything less than 2 years needs x, y, z…).

Here is where my question lies. My dad was paying something like $25 a month into this and claims it was a $25k policy. Does this make any sense? Is a claim for this scenario even worth pursuing?

I have his death certificate, but the copy I have still says “cause of death: pending.” I do have the autopsy report with an updated cause of death (drug overdose ruled as accident). I’m assuming I need to get another updated death certificate? I filled out form A which is his and my info. Form B is for me to provide the names and addresses of all physicians who treated him in the 5 years leading up to his death. I know his hospital but I have no idea where to begin with that. Form C is authorizing release of health info pursuant to HIPAA (wouldn’t that take care of form B?). Finally, Form D is “to be completed by physician.” I have the physician’s name on the coroner’s report, how would I contact him to fill this form? Is that even doable 5 years later? Would he just pull up that record and fill in what was documented back in 2019?

I would really appreciate someone giving me a “to-do list” breaking down what actions I should take for this. I live in California in the same county where my dad passed away. I don’t know if this would make any difference, but my dad was a Kaiser Permanente patient but was cared for and pronounced deceased at John Muir Medical Center.
