Physicians statement when trying to claim life insurance

I (21F) had my mom (57F) pass away in February. She had 2 life insurance policies for me. One was with triple A and the other prosperity life insurance. Triple A’s life insurance claim form was very simple and I was able to receive the payment easily.

Prosperity life is asking for a physicians statement and also have the document be notarized. I honestly don’t know how to move forward with this. They also want detailed information about how she died and the past doctors and hospitals she has been to in the past 5 years. I was able to fill it out partially but it wasn’t easy for me mentally.

My mom was very private about her health because she knew she was sick and didn’t want help. I have no idea who her family doctor was and the hospital she passed away in is 2 and a half hours away. I’m going to try to go to the doctor she mentioned but that was when she was kind of loopy so I’m not sure it’s even right. I also can’t find much information about her doctor because she saw a litany of them. I’m mainly posting this because I have no support system and even just one person giving me advice or encouragement would be helpful. Has anyone else had to deal with giving this much information to a life insurance company?
