Need help with answering questions for an accelerated term life insurance quote.

I (30m) would like to apply for accelerated life insurance that avoids a physical. For the most part I will qualify based on general questions they will ask, but I have two issues that I would like to know how to best answer in the phone interview.

I do not have consistent annual physicals due to moving frequently and the hassle of obtaining a PCP. I had worked with a doctor before as I had some elevated blood pressure readings, and I was not diagnosed with high blood pressure and was able to naturally get my readings to around 130/80 range. Is this worth noting in the interview? I'm sure I will need to grant access to my former PCP records for underwriting.

I was charged with a DUI in 2014, but the case was eventually dismissed with prejudice after completing first offender program. I have read some questions ask have you had a DUI in past five years? Have you been charged/committed a crime in past ten years? Im not sure how the case shows up in my criminal record when life insurance will pull it. How should I address this and will this point alone generally disqualify me for accelerated underwriting?

Other than the above, I have no other red flags or concerns with questions, so I'd like to have a good understanding before beginning the process. My understanding is this information is shared with other insurers as well, so I need to get it right the first time.
