Why would my insurance cover some PT sessions but not all?

Hi there! Sorry if the flair is wrong, by the way…

So I recently started going to PT for some issues with my neck. I have been there a total of 8 times since June 17th and still have 6 sessions left in my round.

I got a bill from the PT yesterday for $190!!! I was extremely confused as I thought that my husband’s insurance (PPO with a Park District) would have covered it minus a copay. I do give them a $5 copay per session so that’s typically a sign to me that insurance has taken care of it.

Since then I logged onto my insurers website to look at the claims and, to my surprise, the first 3 sessions of PT we’re not classified as “fully paid” but rather as “discounted” which left a big chunk of each session for me to pay. It totals to about $500.

Thankfully each session after those first 3 were fully covered by insurance and are already processed. However I am very confused by this.

Is this a question for my PT or for my insurance? Any help is appreciated! Thanks!
