Who do I go after and how? Insurance of doctor?

Back in March I went to the ENT to clean out my ear. My copay was $100 but they charged me nearly $200. Something about paying upfront. I asked them wtf and they said insurance will take care of it and not to worry.

I went back again the following saying that I was experiencing vertigo after the clean out. They cleaned it some more and they gave me some drops iirc.

Again they asked for over $100 and "insurance will take care of it".

I get my explanation of benefits and insurance shows that I was only responsible for (and paid) the $100.

So I call the doctor. Their billing department never answers. It's always a machine. I left voicemails for a couple of weeks. Then I emailed to get a receipt, which they sent me.

I sent it to the insurance asking how I get my overage. They told me I have to deal with the doctor.

After more voicemails they call back and said they were having difficulty with my insurance paying the whole thing. As I understand it, they charged me upfront and then charged them. But they way they did it was that they charged part of it to the insurance, and then tried to charge the rest, but insurance denied it as a dupe. The doctor wants that payment or they won't refund me.

Now it's been months. I've left several voicemails with no response.

How do I get my money back? Go after the doctor? Submit something to the BBB? Leave a negative review everywhere I can?
