Where would you get insurance with a lot of pre-existing conditions?

So I have diagnoses of epilepsy, level 1 autism spectrum disorder, GAD, Learning Disorder NOS, Hypothyroidism, Irritable Bowel Syndrome with Constipation, Impacted cerumen, essential tremor, and inverse psoriasis. Since I plan to work after I graduate college, I was wondering where I can get insurance once Medicare no longer gives me free part A. Timeline is about 5 years half-time, if everything goes smoothly. I need these conditions all covered because the care I need for them and the medications are expensive. I do not think I could make enough income to pay for everything and I have no savings. Any ideas who would cover my current conditions? I already have State Farm renter’s insurance, so I’ll probably be asking them, but need other suggestions. I am currently 41 years old, female, and live in Ansonia, CT 06401. Right now, my income is $17568. When I graduate, I am expecting to make at least $90,000/year as a 35 hour/week Sustainability Specialist.
