What is the difference between Choice+ and Primary PPO plans?
I'm have several choices for UHC through my employer of plans. I'm considering PPO plans. I'm in NY state.
One set of plans is called "Choice Plus" and another set called "Primary Plan". The two seem very similar – in terms of summary of benefits, deductible, no referrals, and the monthly premiums. There are some differences in coverage, deductibles, coinsurnace/copay, etc. [Edit] Primary Choice Plus is slightly cheaper in premiums.
It's confusing because even the Primary plan in the summary of benefits says "Choice Plus".
Is Primary network of providers smaller vs Choice+ or something? Or what other differences? Any insights would be much appreciated.

As an aside – I'm looking at regular outpatient mental care and the primary plan has no charges for in-network.
