What insurance plan should I get (Aetna)?

I started working at CVS and I'm struggling really hard with understanding health insurance. I'm currently on MassHealth under my mom as a dependant, but I don't want my income to affect her eligibility so I think buying through my employee is the best option? I'm 22F and haven't been to the PCP in 4 years now. I think I'm healthy, but I've had recurring UTIs for the past 2 years which I go to urgent care for. I get them every couple months, but it hasn’t been as often recently. Minute Clinic also will not treat recurring UTIs. I do want to get some tests done just to make sure I'm healthy, but I'm not sure if that falls under preventative care. I'm thinking about a hybrid plan, but I also feel like I should get a HSP plan for the HSA and the $500 contribution. There are many providers in network at my location so I'm not worried about that. I would appreciate any help or advice!!! Thank you in advance.
