What diagnostic codes would warrant a service provider to bill an eye exam (92014) to medical insurance instead of vision?
I know it’s financially advantageous for providers to bill eye and refraction (92015) exams to medical insurance since it’s often not covered and the reimbursement rate is higher than if billed to vision insurance. As I understand, there are certain diagnostic codes filed with 92014 that warrant the provider to bill medical insurance. What are these diagnostic codes? Are the codes I listed below enough to permit the provider to bill medical insurance?
For example, my recent 92014 and 92015 were billed to medical, and ofc it’s not covered. Hits my deductible and I’m responsible. There are four diagnostic codes filed with 92014. They are H35.411 (Lattice degeneration of retina, right eye), H35.423 (Microcystoid degeneration of the retina, bilateral), H16.143 (Punctate keratitis, bilateral), and H02.881 (Meibomian gland dysfunction right upper eyelid). There is only one diagnostic code filed with 92015. It is H52.13 (Myopia, bilateral).
