Wellfleet Insurance (under Cigna) Denied Rheumatologist’s Blood Test from Two Months Ago

I am under Wellfleet Insurance provided by my university. Two months ago I saw a rheumatologist from symptoms of which I had to go to the ER for (you may find it in one of my recent posts to this subreddit).

Just this Friday, I received a bill from Quest Diagnostics saying that I had to pay in full because my medical insurance (Wellfleet insurance under Cigna) denied the whole blood test. Diving into the Wellfleet EOB, it gave a code “I2823” with it stating that it stands for “Member must complete and return the accident section on their EOB to determine benefits”.

I’m planning to talk to my rheumatologist about this to see what she can do to help me on this, but in the meantime I would like to seek advice on this. As described before on my previous post here, all of the diagnoses and symptoms started from a foodborne illness that I got from frozen Costco pesto from my freezer back in September, which slowly spread over the course of a month and a half to a systemic body issue (progressive muscle weakness, joint weakness/swelling, etc.).

This accident section of the form wasn’t exactly made for these sort of things. It wants exact date and time of injury, the body part injured, the details of the injury/accident that occurred (and where), if a third party is responsible and if it is an auto accident, if it is injury/sports related, I can place the exact date symptoms started since it was right when my first practice test started (and I still have it on my calendar), but I have no idea how to write in the rest of the information given the timeframe involved – and that they .

Any advice how would I fill this accident section given the information above would be greatly appreciated!
