We paid full amount on date of IUI processing but insurance claims processed shows all the amount is "plan discounted"

The IUI provider is a COE (center of excellence) and in network.

– They asked us to pay out of pocket , before the treatment, otherwise they would not do treatment.

– So we paid about 2000$ for three treatments.

The reason to pay out of pocket upfront is they said the pre-authorization from insurance had not been done yet which was true. But now it is done and all the claims are processed. In the claims , UHC insurance say the entire amounts are plan discounted and "amount you may owe = 0" & "amount paid by insurance = 0".

I am confused and afraid that IUI provider is basically making us illegally pay the money upfront and then not giving it back to us when insurance has discounted the service completely (for whatever reason). They said they will give us credit for next treatment, but until now they keep saying the credits are pending because insurance bills are being processed non stop and they don't know what the credit amount is. I feel like they're just spinning a carrot in front of me.

I am woefully ignorant of this and it is causing us great stress because we are going to end up paying more than 20000$ next out of pocket with the same kind of reasons. I feel like the insurance should be the first line of defence here but this provider is bypassing insurance.

They are also making us sign insurance waivers before treatment begins.
