Visits being paid by state and federal funds?

I have been going to a psychologist medication management doctor for over 2 years. At the end of each visit they ask me to sign a form on the ipad. I never really paid much mind to it, I thought I was just aggreeing to pay the bill (I have private insurance). Recently I left a visit without stopping to sign the form. I was peppered with phone calls that began as I was leaving the parking lot and in the following days that I need to sign this form or they won't schedule any more appointments. They said it was loaded on the portal and I could sign it there. I went on the portal and read the form:

I certify that the information shown on this invoice is true, correct, and accurate. I understand that payment and satisfaction of this claim will be from federal and state funds, and that any false claims, statements or documents, or concealment of material facts, may be prosecuted under applicable federal and state laws

I saw no reason to sign this since my visits are paid with private insurance, so I did not sign it.

Fast forward to yesterday and I got a call from the office insisting I sign it or no more appointments can be scheduled. I explained to the person why and she said basically you have always signed it before….well that was my error and I am not signing anything that is not true. She got very updet with me and is still insisting I sign it.

I am starting to wonder what is going on here. Are they getting grants or some kind of funding from having patients say their bills are paid with state and federal funds? I just can't wrap my head around why they want me to sign a form that is untrue. Any insight? I am in PA and this office has 2 locations.
