Unsubmitted claims provider not cooperating

hello,
I hope someone here can help me I am very frustrated about this situation. I have a therapist who is in network who I have been seeing for quite some time. I have had 10 appointments with her this year she submitted 1 claim in February and none of my other appointments were submitted however she has still billed my HSA card.
I am working towards my deductible which should have been reached by these appointments so my HSA is maxed out however my deductible is 1500 short. I have brought this up to her weeks ago and she has moved very slowly in rectifying this. I nor my husband can take appointments until this issue is resolved.
She finally submitted the claims but due to them being so late United Health Care denied them and are requiring further documents. I feel so stuck because she is not making this a priority. I can not afford to pay any appointments out of pocket and I have urgent appointments that I can not go to. I know she will drag her feet sending the documents if she even sends them.

Does anyone know how I can go above her to get this resolved quicker myself?
