Unknown place is billing my insurance.

A few months back, I received mail from my insurance stating that they were processing my appeal for (service date, location). But I did not receive any treatment on that date, or on that location, at all. Never been anywhere near it. Since then, I've received multiple similar mails, insurance is processing my appeal, same location, different service dates about half a month apart.

The first time it happened I chalked it up to the industry practice of addressing the bill from an affiliated physician or location (though in retrospect, checking my google maps timeline, I was at home the whole day on the supposed service date). For the second one I didn't notice it was for a different service date, I thought it was just the insurance telling me they were still working on it. But at the fourth or fifth, I went into the insurance website, and I have about 10 pending claims from the same facility, with service dates about half a month apart from each other.

I know for sure that I have not been to the doctor every couple of weeks. I suspect fraud. I called the insurance and told them the situation, they are filing a grievance and a fraud report. Could it be something other than fraud?

And if it is fraud, what do I do now? I don't think the facility has my address (I have not received any bill from them), but that doesn't mean they won't be trying to collect somehow. All the claims they have processed from this facility have been denied as out of network and so far they have billed a sizable amount.
