Unexpectedly stuck with full MRI bill

I've been seeing an Ortho in Denver about my hip problems. I have great health insurance (or so I thought) and everything has been covered. I've done Xrays, PT, all covered. They told me I need an MRI, which I did, and then randomly 1 week after the MRI I was given a bill for the full cost of the MRI.

I talked to my insurance company and they said they didn't cover it because the person was out of network (despite facility being in network). I called my doctor and asked why I wasn't told I'd be responsible for the full MRI bill and they were surprised by that, told me United had authorized it and gave me the authorization number and dates to take to United.

I called United and they gave me the same reason again, and told me the authorization number they have is totally different, then put me on hold for 15 minutes and randomly disconnected the call.

I can afford the bill, I'm just annoyed cause this seems wrong, any thoughts?
