UMR: Just received a bill for $5k from a hospital visit back in Sept 2023 because insurance requested refund

I’m from Nevada. I have UMR under my dad’s employment. Our plan covers emergency room visits for $300 copay. At the time, we had already met our deductible for the year. I went to the hospital for a pretty bad car accident I was in (September 2023) and all they did was an MRI. I never got a bed and sat in the waiting room for 6 hours. I did my MRI and eventually got called back and sat on a chair near the entrance of the hospital ward and was told my results. That was about it.

My initial bill was around $12.9k. As I said previously, my insurance allegedly covers everything aside from the $300 copay, which I paid back in March when they initially billed me.

I ended up suing the guy who crashed into me because my car was totaled and received a settlement of $25k. Over $12k went towards my pt, hospital bill, etc. My lawyer and I both received ~$7.3k. My lawyer explained that $5k would be going directly to my insurance and the other ~$7k would go to other PCPs and something else I don’t remember.

Well, fast forward to Monday (8/26/24). I receive a bill in the mail from said hospital for $5k. I called the billing department and they said that my insurance has requested a $5k refund. I’m not sure what my options are. I’m trying to reach my insurance but they have been passing me around in circles. I tried reaching my previous lawyer to no avail.

I only gave until tomorrow (8/30) to pay in full or request a payment plan.
