Trans, Undocumented, and turning 26

Hi, I have a very specific question and I'm hoping to get an idea of what to do. I currently live in Chicago, IL.

For the past 2 years I've been trying to get phalloplasty through a specific hospital in Illinois. I've currently had my pre-op appointment and have another appointment on January 2025. Presumably if everything goes well, I'll finally get my surgery date. I have no interest in starting over at a different hospital. I trust the surgeons there and don't want to wait another 2 years. I was very lucky to have my father's health insurance, unfortunately, I'll be turning 26 on 12/20/2024. This means that I need to find a new plan. I can't get any type of subsidized Healthcare since I'm Undocumented (work visa) or employer Healthcare. My only option is private Healthcare.

My main goal is to ensure that transgender Healthcare is covered and that I'll be able to go to the specific hospital and see the specific surgeons I want. How do I go about this?

I found a list of accepted health insurance for that hospital but I know insurance can be finicky. I like one plan on the list it's within my price range but it's a hmo. How do I check that transhealth care is covered by the hospital I want.
