I got a COVID/flu vaccine last year around September and learned much later that the pharmacy there filed the claim with an old insurance that wasn't covering me. Weirdly it went through with no copay, so I thought there was no issue until June of this year when I got sent to collections for ~$500 (for myself and my wife, both covered under her employment).

I spoke with the pharmacy provider, and an individual there said they'd speak with my current insurance company and get it sorted out. I should've gotten a check about a month ago according to him. I feel like I dreamt this call because since then, the incorrect (old) insurance has removed record of the claim from their website and the provider has no record of my call with them.

Going through my new insurance for reimbursement today (again) wasn't helpful because they need proof that the claim was attempted within 90 days, but the old insurance company has suddenly erased the claim. I wish I'd screenshotted the claim information from the old insurance company's website. It was what I used to fill out the reimbursement form with my new insurance a little over a month ago.

Does anyone have any advice on how to proceed? Not sure if this money is just lost to collections now, or if collections itself was a scam. I don't think so because the old insurance company had evidence of the claim up until I spoke to the pharmacy that provided the vaccine. Feels like someone is covering for someone? I spoke to the provider today and they said the claim went through with the old insurance no problem in their records, but that insurance's coverage ended four months prior…so that shouldn't have been possible. I have no idea how the record of the claim disappeared, but it's more likely to be incompetence somewhere.
