Question about No Surprise Act Balance Billing

My insurance through my employer’s self-funded plan led me to believe that a surgery facility for a pre certified planned surgery was in network which I had verified through NPIs and service address with the insurance service team prior to the date of service. They subsequently processed the claims for the surgery center and anesthesia as out of network. Apparently the insurance team gave me incorrect information and through an appeal I got them to reprocess the claim at the in-network benefit level. Although they re-processed the claims at the in-network benefit level, they only paid the anesthesia at Medicare rates and now the anesthesia office is balance billing me. Can I use No Surprise Act or anything? The anesthesia office says they can still balance bill me even though my insurance plan gave me incorrect info about the surgery center’s network status. The insurance team is saying they’ve paid everything and I shouldn’t owe anything. Feels like I’m getting stuck with the balance bill even though the insurance plan made the mistake giving me bad info and now I have no protections under no surprise act. The balance bill is like $1.5k which is not high enough to justify an attorney but it’s still a significant unplanned amount I don’t think I should be responsible for.
