Provider knowledge of insurance

So in 2023 I was still on Medicaid but I was transitioning to private health insurance. However, the medical provider was able to get all of my new insurance information even though I never gave it to them and I never informed my new insurance that I was going to seek care at this specific medical facility. If anyone knows how they got this information, please let me know.

Fast forward to now, I am going to a different medical provider and going to be using self pay as it's much cheaper than going through my insurance. I never told this new provider of my insurance but since last year the previous provider was able to get this information ( no idea how they got it) I'm scared this new provider might get it as well. I know I can legally do self pay, but I'd rather they just not know as they make it much harder if they know I have insurance. Any thoughts on how I can prevent my information being shared like this? I called Anthem and asked if they automatically share my info, but the rep sounded like she didn't know what she was talking about.
