Provider following directions but insurance still denying claim

Insurance is denying a claim saying provider isn’t sending the claim to the correct BCBS.

Tons of 3 ways calls between me, provider, and insurance. Provider is saying that patient is responsible for balance but insurance is saying no, you’re just not sending it to the right place.

Provider confirms with insurance how to send it electronically, but it’s still denied.

So provider confirms an address to send a paper claim, and it’s still denied.

EOB states in-network, $0 patient responsibility, and that it needs to go to xyz.

Provider sends to xyz but it’s denied still, for the same reason.

This has happened 8 times, and I keep getting the same runaround. Luckily, I’ve been able to get the provider to stop trying to collect from me. But I can still see the denials.

Basically insurance is blaming provider and provider is saying they are following insurrections, and I’m caught in the middle.

What gives?
