Procedure initially approved now receiving Initial Adverse Determination – Denied not medically necessary?

Hi everyone. I should have known it was too good to be true when my dermatology office said they spoke to my insurance and a mild chemical peel for acne scarring would be approved. I said really? And asked several times that the doctors office was able to get approval before going through with the procedure and they confirmed yes, it would be covered. In full! I was shocked because when I’d gone over my own insurance benefits it said it would not be covered but trusted the doctors office that they were able to get approval. 3 months later, I received Initial Adverse Determination – Denied not medically necessary paperwork in the mail from my health insurance provider, and it says the service billed was $400 and they won’t cover any of it. How is this legal? I will talk to the doctors office when they open on Monday but just looking for some advice about how to proceed if the doctors office tells me it’s not their fault. Even though my acne scarring is bad, I never would have gotten this done if it wasn’t covered! I feel so stupid. Any advice helps please and thank you.
