Primary care charging me double? PA

I live in Pennsylvania, my work sucks for lot of reasons, one being I pay for my own health insurance. Everything was as fine as it could be until I showed up for a scheduled doctors appointment and am told my copay had suddenly doubled from $40 (Tier 1) to $80 (Tier 3). I ask how that’s even possible and she points to a sign on the wall saying as of (about 2 weeks ago at the time) members with my Health Insurance will be moved from Tier 1 to Tier 3 because my primary care doctor was bought by PennMedicine. I checked my email and couldn’t find any notice of the change and hadn’t received and physical mail either, but lady says I can’t see my doctor unless I pay. I’m in a bad enough financial situation as it is, I had been in a car accident the day before and I was out of a med that I could not get a refill of without this appointment so I sucked it up and paid. Now I got a bill in the mail saying I owe Penn another $25 for the same appointment I already paid for. I can’t afford to deal with this. Who do I talk to to get my payments back to some semblance of normal?? Do I need to find a new primary? A new Insurance? What do I do???
