preventative care incurred another regular office visit bill

Went to a preventative annual physical exam. The doctor asked for my medical history. So I told her that:

I have internal hemorrhoid;

My ankle had fractured 3 years ago;

I used to have low levels of Vitamin D 4 years ago; but after taking the supplement, it was back to normal for the last 3 years, so I want to continue monitor it.

After the visit, the doctor billed me $400 for the preventative care and $350 for the regular office visit as she discussed the above 3 concerns with me.

But my point is, I don’t need any info/input from the doctor on these concerns:

My internal hemorrhoid were managed by a specialty doctor very well; I know what’s happening and how I should manage it;

My ankle fracture is also managed by orthopedic doctors.

My Vitamin D levels were already normal for the last 3 years; I mention it just want to make sure the doctor will check VD levels during this preventative care.

Is it legitimate for the doctor to bill me an extra office visit? I really don’t need any input from the doctor on these issues; I mentioned these just because the doctor asked my medical histories.

If it is legitimate for the doctor to charge, should I refuse to say anything in my next annual exam to avoid these ridiculous bills?

The doctor already charged $400 for the preventative care; why they still so greedy to charge an extra $350 for just telling me info that I don’t need and I already knew (like eat more fibers, etc…).

I already disputed this bill with the provider and filed a BBB complaint. Are there any other ways to dispute or complain if the provider refuse to withdraw this bill?
