PPO or HDHP for Chronic Illness?

Hi there! I am signing up for health insurance at my new job and have a few questions regarding the best plan. I am in my early 30s and have rheumatoid arthritis as well as a neck and wrist injury. I take a multitude of daily prescriptions, see specialists and my GP regularly, and also take a monthly Cimzia injection (biologic). I know the general notion is to choose an HDHP if you are young and healthy and to choose a PPO if you have a great deal of health needs – unfortunately at this point I think I fall into the latter category. These are the options offered by Blue Cross Blue Shield:

HDHP “HSA $3200 Buy-Up Plan” $3200 deductible $69.37 monthly premium $6400 in network out of pocket limit ($10-35 for drugs and 20% payment for care)

HDHP HSA $5000 Base Plan $5000 deductible $43.77 monthly premium $6500 in network out of pocket limit ($10-35 for drugs and 20% payment for care)

PPO $2000 Plan $2000 deductible $100.90 monthly premium $6000 in network out of pocket limit ($20-40 for drugs and $30 payment for care)

I’ve studied and read articles and weighed options and I still don’t know what to do. Can someone please give me some advice?
