Possible Medical Fraud – Unsure How to Proceed

Recently, I discovered two claims on my insurance account from a provider called Maxcare Diagnostics Lab for COVID-19 tests. Here's the catch: I never took any COVID tests during that period, and to top it off, the lab is located in a city that I don't even live in.
I've tried reaching out to the lab multiple times, but my calls are met with radio silence. I also contacted my insurance customer service, only to be told that they can't get in touch with the provider either. I was told that I needed to wait for the insurance resolution team to reach out to me with further instructions. And because I didn't receive bills, so I can't file an appeal either.
Does the absence of bills mean I'm okay to wait for the insurance resolution team, or should I be taking more proactive steps?
Has anyone here ever faced a similar situation or have insights on how I can protect myself from potential fallout? I'm concerned about the impact on my insurance and the fact that my name is associated with false claims. Thanks tons in advance!
