PCP refers me to PT who refers me to PT??

I have terrible insurance (Metroplus), and it has proven near impossible to get a physical therapist in NYC. Every time a representative found me one, it turned out they no longer accepted Metroplus or the doctor had left the practice. I literally sat on the phone for 45 minutes while a representative called PT offices in order to get me a doctor and ultimately failed, but kept insisting that I can do this myself by using the same website she was using.

So, eventually, after dedicating an entire day to it, I find a physical therapist on my own and consider myself lucky. I'm expecting to go in there and start rehabilitation. Instead, I arrive and this PT is more like a primary care doctor. She looks me over, orders X-rays for me, and then writes me a referral for a physical therapist. I ask her if she can recommend one. She can't. Tells me to ask the front desk. The front desk hand me a flyer for a PT group that they presumably give to everyone, and it's out of network for me.

I have no idea what to do at this point. I'm considering reporting Metroplus for failure of Access to Care or trying to appeal for the right to an out-of-network provider as mentioned here:

That said, any advice would be greatly appreciated. Also, is there a difference in title between a PT doctor who examines you and one who actually helps you with rehabilitation?

TLDR: Insurance can't find me a physical therapist for rehabilitation. Not sure what to do.
