[PA] employer insurance plan raising premiums based on specific criteria

Got a notice that all those under any of the plans must go to a doctor this year by a certain date and “pass” 5 out of 8 criteria or the persons premiums will increase next year. If they do not pass they will have a grace period until Jan 2023 to repass and meet the criteria. They criteria are as follows:

A1C < 5.7%

Blood pressure < 120/80

BMI of 18.5 – 24.9

Total cholesterol < 200 mg/dL

HDL > 40 mg/dL

LDL < 100 mg/dL

Triglycerides < 150 mg/dL

Waist circumference (female < 35 inches) (male < 40 inches)

Is this legally allowed?

Does it matter if it is the insurance company raising premiums due to this criteria or the company cutting back on what they cover based on this?

I’m already gonna pass so it makes no difference to me but this seems like something that is not allowed since I thought there were legally set requirements for raising premiums as well as reasons why you are not allowed to raise them, such as preexisting conditions or weight.

Company is also not located in my state, main headquarters in Wisconsin, but has employees across the US.
