Out of network doctor, in network facility (Houston, TX, United Healthcare Marketplace/Kelsey Seybold insurance) for ovarian surgery

1) Fiancee has an ovarian surgery scheduled next week with her OBGYN she likes, but is out of network. She scheduled her first appointment with her because the Dr. worked at the in network hospital and she didnt know. Doctor's fees are pretty low for us- so we didnt care (~$100/appointments, $1200 for physician fee for surgery)
2) The hospital (st lukes) is in network.

3) The hospital has completed preregistration for her and said the facility fees should be covered under insurance

4) We have United Healthcare Marketplace coverage (Kelsey Seybould), which has mostly the Kelsey Seybould clinic doctors listed but several local hospitals also listed as facilities.

Question: Is it likely the insurance will deny coverage for the entire claim because the doctor is out of network?
We don't mind paying the physician fees (which have already been collected [$1200].

Yes we will call insurance on Monday to verify.
