Optometrist keeps billing annual exams as medical because of a previous history

I have medical insurance through Cigna and also vision insurance through VSP.

My annual vision exams have always been 100% covered through VSP. However, last year, the optometrist and i were discussing a past retinal year I had (7 years ago) and what kind of vision floaters to took out for. Because of this, the office billed this as a medical appointment to Cigna and I had to pay $100 out of pocket. This year, I made a mental note not to ask any questions regarding my past retinal tear, and the only time it came up was when the optometrist asked about past conditions in their questionnaire. Maybe it came up in conversation about my past history but I very consciously did not discuss it. My eyes were not dilated so the optometrist did not even look at the tears. However, it was again billed as a medical diagnosis to Cigna and I am having to owe $100 again. I’m just frustrated because I’m not sure what I’m doing wrong (if anything). I never had this issue in the past. Should I just pretend I’ve never had a retinal tear? Refuse to acknowledge any mention of it? Do I just have to accept that I need to pay out of pocket every time?
