NY Empire Plan for Retirees – doesn’t cover co-pay for skilled nursing care. Need to learn options for 94 y/o

A relative was just handed a huge bill for their $200/day co-pay for 50ish days of skilled nursing care, despite the fact that they have Medicare A & B; The Empire Plan for NY teacher retirees and a major medical insurance. I contacted NYSHIP and the school district that they worked for – and all state that Medicare pays for the first 20 days in skilled care but that after the 21st day the co-pay for skilled nursing care is not covered. Their retirement benefits pay for their medicare premiums; they pay a premium for the major medical, but don't have out of pocket expenses for the other insurances, other than co-pays. How can I find out what other affordable insurance options might be available that have better coverage for someone who is elderly and has numerous chronic conditions; and is now recovering from acute illness. They live in Maryland, but retired in NY. If they drop their Empire Plan – they can't ever return to it.
