No suprises act question

I just got a $788 bill from my psychiatrist and I haven’t had an appointment with them since June. This bill is for 3 appointments, 1/2023 6/2023 and 9/2022. This is the first bill I’m getting for these appointments and my copay is usually $25. It seems fairly likely that getting sent a bill from just over a year ago could be illegal as I can’t send it to my insurance to even ask if it can be covered. I’ve always had issues with this office as they have switched my provider 3 times with no notice. Doesn’t help that they wait to tell me on the very day I run out of my prescription, so I just have to book another appointment instead of being allowed the time to find another doctor that will work well for me. On the bill it also states that my appointments are 30-39 minutes when I’ve never had an appointment longer than 15. Is there anything I can do? Or do I just have to pay it before it goes to collections. Any help will be greatly appreciated as it’s the end of the month and I need to make rent.
