Newborn has balance due 1 year later, Insurance never charged, never received a bill

Hoping someone here can help with some information..My now wife and I had a baby almost a year ago and at the time were unmarried (covered by different work provided insurance plans). At the time of the birth I provided my insurance information to the hospital as our baby would be covered under my insurance. My insurance was billed for certain services for our baby such as the pediatric visits in the hospital so we assumed all was good.

However, my wife has been receiving cryptic notices from the hospital on behalf of our child saying that a claim was submitted to 'Commercial Indemnity Prim' (not either of our insurances) with a polciy # that is neither of our policy #'s and that it was denied and that we should reach out to her insurance. Neither of our insurance companies denied any claims that we can see so I'm not sure who this claim was submitted to. These notices say no payment is required and that a bill will be sent. However we have never received a bill.

The hospital finally called now almost a year later and tells us that there is an outstanding balance on our child's account for $3,500 for my child's hospital stay. They say they didn't have the insurance information to submit the claim (we have other charges billed to my insurance in my child's name from that time so this makes no sense). They offered to charge us an insured price of like $350, but we still have no idea what we're even being charged for…we've asked for an itemized bill to be sent. I'm assuming at this point insurance will deny this based on the fact almost a year has passed. What should we do here? Just pay the uninsured price? Is this even legal?
