New to US health insurance: PPO vs Buy-down CDHP vs Standard CDHP

I am new to the US workforce, and this is the first time I need to select health insurance. As you can imagine, this is very complicated as someone who hasn’t grown up here. My company provides three different options that I am trying to figure out what is the best balance between cost-effective and peace of mind when talking about health costs.

A bit about myself: male, early 30s, I have asthma (currently using medicine the I brought outside of the country but will need to eventually figure this out here), some lower back pain that sometimes requires me to go see a doctor. I workout 4-5 times/week consistently and like to keep an eye on my health metrics, so I try to run physical/blood check-ups 2x year. Same goes for dermatology, I am going through some treatments and see one every quarter.

PPO gives me access to HCFSA.

Buy-down CDHP gives me access to HSA where employer contributes $350.

Standard CDHP gives me access to HSA where employer contributes $600.
