My kids and I haven’t been covered in months

My two kids and I have been under my husband’s insurance. Around the beginning of this year, we noticed that the Anthem app said my husband was covered, but myself and my kids were not covered anymore. He went to his work and asked, they took a look, and told him that everything looked good and we were covered and good to go.

Over the last two weeks, we’ve had a lot of appointments grouped together. Check ups for the kids, dentist appointment, a check up for me, and an ER trip. Discovered that none of these appointments were covered and we owe the full amounts. Checked the app again, it shows we still aren’t covered. My husband has had the full cost of insurance for all of us taken out of his paycheck this whole time. $300 a week.

Now obviously I know we should have checked the app again ourselves at some point and we are dumbasses for not doing so, but the point is my husband has been paying thousands of dollars for coverage that doesn’t exist. Would his company be on the hook for back pay? Or do we just have to suck it up and eat the cost?

TLDR; husband paid for months of health coverage for 4 family members through his work—3/4 of the family isn’t covered.
