Moving from CA back to NV and depend on medical infusions every 5 weeks. Where do I start?

I am currently a CA Kasier Permanente member through my employer and have been happy with my plan for many years. I recently resigned my position here in CA and I am moving to NV at the end of this month. My current Kaiser plan is active until the 30th of May but my former workplace will allow me to stay enrolled month to month if I wish although that will be inconvienient since I would have to travel back to CA everytime I want to use my benefits. The only reason I would even consider that is because I undergo medication infusions every 5 weeks that are the foundation of my quality of life and a lapse in coverage is simply not acceptable. Ill fly out to Orange County for infusions every 5 weeks if i have too! I was hoping to get some feedback about where I may start my research regarding setting myself up for continuity of care in Las Vegas. Financially, the infusions are important enough that I would be willing to enroll with COBRA if that would make the process of getting in with a new doctor simpler or I could prove a qualifying event and enroll in a new plan on the open market from what I understand. Is this a job for an independant broker or a similar type professional? Any feedback is greatly appreciated!
