Medical records — what should you get?

A family member was in a Skilled Nursing Facility, and was informed with three days notice that Medicare would not allow ongoing services. A Livanta appeal was quickly denied.

The patient submitted a signed request for medical records, and today they arrived, accompanied by a disclaimer stating that they had been sanitized by the legal department (This SNF is owned by a private equity firm). The notes include, almost exclusively, copies of boilerplate documents she signed and info that was already available via her regular care provider's portal. The records do not include SNF staff assessments or a copy of the documents sent to insurance/Livanta which resulted in the denial.

What can a patient normally expect to receive via a medical records request? I have been trying to research this online but getting definitive answers (or any direction) seems to be impossible.
