Marketplace/Medicaid helping cover Lifetime membership?

So I’m about to be self employed and don’t anticipate making any money for the rest of the year. I anticipate reporting ~$27k for gross income in 2023, and it seems there are some nice options to get most of or all of my premiums covered through bronze/silver plans on the Marketplace. Medicaid may also be an option but still need to see if I’ll qualify (AZ).

I am looking for a company/plan that provides a low premium, high deductible, and most importantly, some “wellness” benefits that would help me justify becoming a Lifetime fitness member ($200/mo) by lowering costs in any capacity.

By “wellness” benefits, I’m referring to 2 things:

1) Third party administrators that partner with Lifetime and reduce monthly costs (Gympass, One Pass, Peerfit Tivity Prime, Active&Fit Enterprise), or policies that provide credit that’s eligible to be used for gym memberships

2) Policies that provide generous ($500+/year?) contributions to HSA/MRA/FSA accounts (I have a letter of medical necessity for the gym)

Based on my research I’ve seen a couple possibilities:

-AZ Medicaid, UHC Community has a lot of wellness credits -BCBS AZ has some bronze/silver plans I’ve seen on the marketplace that appear to have generous credits

Any advice would be very much appreciated.
