lost COBRA, any private PPO options? (NY)

Hi, I am in a complicated situation and I'm not sure what to do. So until recently, I have been covered by a PPO plan via COBRA after being laid off in May. I recently lost my COBRA coverage due to nonpayment. I am trying to get reinstated (i thought i had autopay on :c ) but it looks like they are unlikely to reinstate me. I might be eligible for medicaid or a marketplace plan, but the issue is that I am trying to get a medically necessary surgery with a surgeon who is out of network with all plans. I was wondering if there is any way to buy a PPO plan directly from an insurer until I get a job, so that I can continue with my surgery preparations. I understand (maybe incorrectly) that if I don't buy insurance on the marketplace, it could be non-ACA compliant which means I will not be covered for pre-existing conditions. Do I have any options whatsoever in NYS besides getting a job? (trust me, i'm trying) Thanks!

EDIT: I am 24 (but neither of my parents have plans I can join), no income at the moment, and in NYS
