Issues with UHC – how to report?

I am going to make this as short as possible. I have UHC coverage through my employer. It IA a self funded ERISA plan. I have been trying to fight a denied preauthorization, but it wasn't going anywhere. UHC said to contact an individual. When I did, it turns out she was head of HR for a sister company of my employer. They gave us incorrect contact information and have been very unresponsive. They also gave me incorrect information on how to file an appeal and I received letters that my policy is not covered under those appeal processes, despite that being what UHC told me to do.

My next issue is I started a second policy through part time employment. It covers fertility, which my primary does not. I requested and was told multiple times they would send the COB letter, which they have not, and which I am now being told is not possible. So either UHC is hugely incompetent or they've been lying.

I tried to file a complaint with my state insurance board In mississippi, but since it is a self funded ERISA plan, then it was immediately closed. I am at a loss here. UHC is literally playing with my health and nobody cares except me. Is there a way to lodge amy kind of complaint with any governmental (US) body, or are health insurance companies just allowed to play with our lives without oversight?
