Insurance not applying contract rates?

Hi everyone. I recently went to the ER after cutting my finger open on glass late at night. Urgent care was not an option due to how late it was and I couldn’t stop the bleeding on my own. I had three stitches, a finger X-ray, and a tetanus shot. I also want to preface this by saying I understand I have a high-deductible plan at $5,000, but it’s one of the best plans my company offers and it’s just what I have to work with right now.

My question to you all is if there is anything that looks off about this EOB when it comes to the contract rate application? It looks like insurance didn’t start applying the contract rates until the last 2 line items and it seems like they just allowed the hospital to charge in full up until the $5000 deductible which doesn’t seem right to me. Every other EOB I have through this insurance reflects a discount or reduction based on the allowed amount. The first four line items show nothing at all here.

When I called the insurance company they said those first four items were the allowed contract amount which I’m not sure is true. I wasn’t quite sure what else to say at that point so I ended the conversation. Am I wrong in this situation or should I continue to question this?
