Insurance Company Refuses to pay for MRI

I have a history of spinal surgery for a herniated disc. Before the surgery, I had to go through pain management, steroid injections and physical therapy. Trying all three of these treatments took an entire year. All three of those things did not help me but I had to go through them in order for my insurance company to pay for my procedure that me and my doctor knew I needed. I was recently diagnosed with an spinal stress fracture. It causes extreme pain to the point that some days I can’t stand up straight. There was even a point where I was unable to dress myself or shower. I had an xray and have seen two doctors that both say I need an mri. One of those doctors is a spine surgeon. My insurance company refuses to pay for an mri without me completing six weeks of conservative treatments. I’ve explained to them that I have a history of spinal surgery and none of those conservative treatments worked for me. I was told that they will not pay unless there is a nuerological need which is absolutely absurd to me. What can I do to make them pay for this mri that I need? I need advice. I’m in extreme pain and at my wits end. I don’t want to spend another year of my life trying to prove to these people that I need something that my doctor ordered.
