Identical charge billed to both primary and secondary insurance?

Correct me if I’m wrong on anything or if someone can help with more info. I can give the whole EoB’s if advised.

tl;dr I think the identical charge was sent to both insurances, maybe because of the name which should be the same but is slightly different.

I have the code from both EOBs which was sent to both my primary and secondary insurance. Primary paid it and says I may owe $600 (understandable). Secondary pays it but says I owe $2500 ($1000 deductible + $1500 coinsurance = $2500 max OoP). On my bill from the hospital it only lists the charges from my secondary and they are asking for $2500. This is the only nonzero charge on the secondary insurance EoB.

I think the names of the two charges are different causing it to be charged to both insurances for the same amount. If anyone can tell me who I should follow up with that would be a great help, thank you.
