I paid a bill from a specialist AND just got a bill from insurance. Is that normal?

I went to see a podiatrist last month. In 2 visits, I paid $50 at the counter each time when I signed in. I got a separate bill a couple weeks ago that totaled about $320 (Four separate detailed charges of $53.27, $146.19, $81.42 $40.08). This was for services provided there, including x-rays, consultation, etc.

This bill did NOT come from my insurance. It was through a patient portal with the specialist office. I paid this bill through the patient portal.

I just got an EoB from my actual health insurance now from this same set of visits. It says I owe $330.88 for the first office visit + radiology + Surgery-Bone/Muscle (idk why it describes it like that… I just got my foot taped up, that's it). It says I owe $90.08 for the 2nd visit, and just says that is for the visit itself and nothing else.

So I paid the office through a patient portal $320 for the services, and now my insurance says I also owe THEM $420.96 for the same services.

Is something messed up here?
