I owe $1400 to my health insurance that I never knew about and now I don’t know what to do..

First time posting here. I injured my knee in December of last year when I was on vacation in another state and I went to ER in that state since I was not home. After coming back home I went to my PCP for checkup and he referred me to a speacilist(doc no 1) for further checkup. When I gave the specalist call, the receptionist informed me that doc no ) only sees sports related injuries and not others (I going injured when I fell down on snow). So, the receptionist told me to go to a different doctor(doc no 2)- pretty sure they were in the same hospital but different locations- and recommended him to me. She gave me the number, now I was told to go to doc 2 to get checkout. I called, made appointment and was scheduled for after a month. I went to doc 2 and when I went to the receptionist made sure to ask if my insurance is active and if everything is in place and she told me yes. I went ahead and got checked. He refered me to therapy which i was never able to go because of busy schedule.

Now, its September and I opened my United healthcare patient portal for the first time this year(after renewing the plan- the plan got changed slightly but is under same provider). On my portal it says i owe almost $1400 total as a claim? I am not even sure what that mean. It has 4 different "claims" and it says "denied" and also says that I owe this money.on top of this I never got any emails, texts or even mails for this bill. So I had no idea till now. I talked to customer service rep and he told me to do a appeal. What are the chances that the appeal will get accepted? Why do I owe this money even though I went to refered doctors and made sure everything was right? Any advice would be helpful. Sorry for may mistakes.
