I need help with Appeal letter for $1,700 bill uninsured

Hi, I'm not sure where to ask for help for this so I thought this might be it. Last November I came down with a very bad illness that my family thought was just allergies, urgent care thought was sinus infection. Despite antibiotics it kept getting worse until I needed an ambulance because the symptoms were causing me to have a massive attack. ER told me it was the flu and was untreated and I got meds for it and proper diagnosis. But three days earlier my family's insurance on me ran out. I am 26 and have autism/ADHD and have had trouble the past two years finding work because of my small town and getting a response to applications for a place that can be accommodating for me and my disability plus social anxiety.

The hospital said I owe $1,700 and the insurance i just got can't cover it and my family doesn't want to pay for it. I can't find suitable work and I'm panicking over collect calls now. The hospital sent me a financial assistance packet but I am having trouble filling it out and had to call them to ask for help. They said to write a letter explaining my living situation with my family and work related issues (I've worked several times over the past 8 years but constantly have incidents and social trouble due to my disability and don't have any money left, I live with my family) and that they might be able to lower or erase the bill. But I'm not sure how to write such a letter. I am in Georgia if that matters. I recently was able to get documents from a decade ago proving with an official psych diagnosis I have autism but it's a massive packet of 10 papers and I'm not sure how to include that on the letter.

I also am not sure how health insurance works and if I get a job if it'll posthumously cover this bill? My family kept saying I could get on medicaid but I kept applying on the government website earlier this year and it kept denying me.
