I have a problem with a Quest invoice

Hello, I have a question regarding a Quest invoice. In a medical consultation, my mother was ordered a blood test to which an allergy analysis was added (which was not consented to by my mother and was not covered by health insurance either). When Quest's bill for $2,600 arrived, we had realized than in addition to the fact that the primary doctor did not add medical insurance to the blood tests, he also performed the allergy test without my mother's consent.

I have called Quest Diagnostics and the health insurance, they were able to reduce a part of the total to $700 corresponding to the non-consented allergy test. The Quest representative told me that we should go to the primary doctor and demand that they send Quest a "new diagnosis code, a letter of medical necessity, and an appeal form because the bill has already been processed."

The thing is… the primary doctor doesn't want to take responsibility for that, saying that he has no reasons to have known whether or not that allergy test was covered by our health insurance (ok, that’s reasonable), and that my mom should have known beforehand, not him. But how can a doctor perform a test that a patient says she doesn’t want to because she would like to see other options that her insurance would cover and still add it?

What can be done in this case?
