I have a higher deductible since I am over my "3 non preventive office visit"

I recently got a huge bill that I am responsible for since I am over my "3 non preventative office visit".

I call my insurance company for a list of which doctors did I use for my "non-preventative office visit" because I feel what they have and what I believe my office visits are, are different. Well the person I spoke to over the phone, wasn't able to produce the list because she says there are a lot of claims.

I told her someone must know since they claim I am over my 3 NPOV. After going in circles with her for about 5 times, I just said thank you for your help. Basically nothing was resolved. The person also told me to call my doctors office. LOL. I told her they won't know that I am over my 3 NPOV and to charge me a higher deductible. That is on your end.

If it helps, my insurance is HealthNet. I can't seem to get someone from the USA on the phone. I always get someone from India/Philippines/Mexico. This is my 4th time calling HealthNet and unable to resolve my situation.

How do I get a list from them of which doctors they feel are NPOV? Would filing a complaint to the Dept. Of Insurance help? Also, what if there is a doctors visit that they have and I disagree that it is a doctors visit how will I resolve that? For example, there is an office visit there where I didn't see the doctor but saw the medical assistant to get my blood drawn.
