How to manage pre-existing conditions when switching insurances

Hi, I'm currently on Cigna through Cobra (under my parents), and on November 1st I'm switching to Aetna PPO through my employer. I have multiple pre-existing conditions/meds and an upcoming surgery and am not sure how this transition works.

(1.) I have multiple diagnosed health conditions (e.g., rheumatoid arthritis and a bleeding disorder), all of which require their own medications (like my TNF inhibitor and migraine medication). How will I get all of my meds re-approved under Aetna? Do I need to submit a ton of medical documentation to Aetna in order to document my pre-existing conditions, and *then* manually re-initiate the approval for each med, or do I *solely* let my pharmacy and doctors know that I have new insurance and the rest is handled behind the scenes? I am wondering what *exact* steps to take here, re: meds and pre-existing conditions.

(2.) I have a surgery scheduled for January that's currently in process of approval through Cigna. Should I just tell my doctor to cancel the approval and wait until November 1st to restart it through Aetna? They mentioned they didn't want to even schedule me until I had my new insurance, but I think they forgot and scheduled me anyway.

(3.) Any other tips to make this process seamless?
